Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) — Initial Mental Health Evaluator

Job Overview

Golden Behavioral Health Services is seeking a compassionate, board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) whose primary focus will be conducting initial psychiatric evaluations and establishing individualized treatment plans.

We are a locally owned and operated mobile psychiatric and behavioral health practice. Our providers travel to patients where they reside, including skilled nursing facilities, assisted living communities, memory care settings, and private homes.

This position is ideal for a clinician with strong assessment and diagnostic skills who enjoys getting to know new patients, identifying their mental health needs, and helping establish a clear plan for ongoing care.

Responsibilities

• Conduct comprehensive initial psychiatric evaluations, including psychiatric and medical histories, mental status examinations, and review of current medications and previous treatment.
• Assess presenting symptoms, daily functioning, cognitive concerns, and relevant psychosocial factors to establish appropriate diagnoses.
• Evaluate safety concerns, including suicide risk, and coordinate additional intervention or a higher level of care when indicated.
• Develop individualized treatment plans, including medication recommendations, therapy referrals, and follow-up recommendations.
• Initiate or adjust psychiatric medications when clinically appropriate and educate patients and caregivers about benefits, risks, and monitoring needs.
• Collaborate with patients, families, caregivers, facility staff, and other healthcare providers to gather information and coordinate care.
• Communicate evaluation findings and treatment recommendations to the team responsible for ongoing medication management and therapy.
• Complete timely, accurate clinical documentation that supports assessment findings, diagnoses, medical necessity, and treatment recommendations.
• Provide patient and caregiver education regarding psychiatric conditions, treatment options, and available resources.
• Conduct telehealth evaluations when appropriate.

Required Qualifications

• Graduate degree from an accredited Psychiatric Mental Health Nurse Practitioner program.
• Current board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
• Active Utah APRN license, or eligibility to obtain licensure before starting.
• Strong skills in psychiatric assessment, differential diagnosis, psychopharmacology, and treatment planning.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Proficiency with electronic health records and timely clinical documentation.
• Strong communication, organizational, and time-management skills.
• Valid driver’s license, reliable transportation, and willingness to travel between patient locations.

Preferred Qualifications

• Preferably one year of psychiatric clinical experience.
• Experience conducting initial psychiatric evaluations.
• Experience serving older adults and patients with complex medical and psychiatric needs.
• Familiarity with skilled nursing, assisted living, memory care, or home-based care.
• Experience evaluating patients with dementia, depression, anxiety, mood disorders, psychosis, and behavioral concerns.

Pay: $120,000.00 - $150,000.00 per year
