We are seeking passionate and motivated Physical Therapists to join our growing neurorehabilitation team. In this role, you will provide individualized therapy services to patients recovering from neurological and catastrophic injuries in their homes and communities.

This position offers an exciting alternative to traditional outpatient, inpatient, or skilled nursing environments by allowing therapists to work directly with patients in real-life settings where mobility, balance, endurance, and functional independence are most important.

Responsibilities

  • Evaluate and develop individualized treatment plans.
  • Provide evidence-based physical therapy interventions.
  • Address gait, balance, mobility, strength, endurance, and fall prevention.
  • Collaborate with Occupational Therapists, Speech-Language Pathologists, Case Managers, Physicians, and Rehabilitation Specialists.
  • Educate patients, families, and caregivers on treatment goals and progress.
  • Document evaluations, treatments, and outcomes in a timely manner.
  • Participate in interdisciplinary care planning and team meetings.
  • Assist patients in achieving personal, functional, vocational, and community reintegration goals.

Qualifications

  • Current, unrestricted Physical Therapist license in the state of practice.
  • Minimum of bachelor’s degree in Physical Therapy from a college or university with accredited Physical Therapy program
  • Ability to travel locally within assigned territory.
  • Demonstrates knowledge of rehabilitation techniques related to complex neurological injury
  • Background in home health, acute or post-acute settings a plus
  • NCS (Neurologic Clinical Specialist) certification a plus
  • Experience with home or community-based settings.
  • Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance state regulations
  • Communicates effectively and professionally in verbal and written interactions
  • Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
  • Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
  • Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
  • A health screen or examination may be required prior to assignment and periodically thereafter, depending on the specific location policy, local or state regulations to verify that he/she is physically capable of performing assigned duties with or without reasonable accommodations
