Requirements: Active Colorado Physical Therapist license Current CPR certification Reliable transportation Home health experience preferred; new graduates welcome

Responsibilities: Evaluate and treat geriatric and home health patients according to individualized plans of care Provide therapeutic interventions, including modalities, manual therapy, exercise prescription and trigger point therapy, to improve mobility, strength and function Teach patients and caregivers home exercise programs, safety strategies and discharge plans Document findings and progress clearly in the EMR, following clinical documentation standards Monitor patient progress, adjust care plans, and keep the care team and discharge planners informed About Rehab Accomplished: Rehab Accomplished is a therapist-owned, fast-growing home health agency.
