**Essential Duties and Responsibilities:**
· 1\. Completes Evaluations, Assessments, and treatments accurately, including all documentation/records, according to policy and the individualized care plan in a timely and accurate manner.
· Follows established policies and procedures as related to the scope of practice.
· Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
· A physical therapist gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psycho\-social functioning.
· Plans and delivers effective Patient and Family Training to ensure proper patient care.
· Prepare and submit clinical notes in a timely manner.
**Qualifications/Requirements:**
- One to three years’ experience as Physical Therapist preferred
- Other certifications may be required for this position
- Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills.
**Education:** Graduate of an accredited School of Physical Therapy/ Occupational Therapy/Speech Pathology with a Baccalaureate or Master’s Degree.
**Li censure and Certifications:** Must have a current li censure by the Georgia State Board of the specified therapy and a valid CPR certification. Must possess a current valid Driver’s license.
