What you'll do

• Evaluate patients in their homes and deliver PT services under the physician's plan of care
• Assess functional status and limitations in activities of daily living
• Build individualized exercise programs and help patients regain independence
• Recommend equipment and home adaptations
• Re-evaluate progress regularly and adjust care as needed
• Supervise and direct PTAs and home health aides per regulatory and agency standards
• Coordinate with physicians, nurses, and other disciplines, and make referrals when needed
• Report changes in patient condition promptly
• Complete visit documentation within 48 hours

What you'll need

• Active Michigan Physical Therapist license (new grads welcome)
• Graduate of an accredited physical therapy program (CAPTE-accredited or equivalent; internationally trained PTs meeting federal requirements welcome)
• Home health or clinical rotation experience preferred
• Valid Michigan driver's license and reliable transportation
• Willingness to drive in all weather conditions
• Flexibility to work some evenings and weekends as agreed
• Physical stamina to perform in-home therapy
