Position Summary

Nurse Practitioner or Physician Assistant to provide high-quality care for long-term care residents by assessing, diagnosing, treating, and coordinating care alongside physicians and the interdisciplinary team.

Key Responsibilities

Conduct adult/geriatric assessments and physical exams Manage acute and chronic health conditions Collaborate with physicians, families, and care teams Support care transitions and treatment planning Participate in quality improvement initiatives

Qualifications Active NP or PA license in the state of practice Board certification (AANP, ANCC, or equivalent) Minimum 2 years of clinical experience Active DEA license Current NPI and Medicare provider enrollment CPR certification
