PRN/Contract Home Health Physical Therapist – Northern Virginia

We are looking for a motivated Physical Therapist to join our growing home health team on a PRN/contract basis.

Provide skilled PT in patients’ homes, helping primarily older adults improve strength, balance, gait, mobility, safety, and independence.

What You’ll Do

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Educate patients and caregivers on safety and home exercise programs
• Coordinate care with physicians and other healthcare professionals
• Complete timely and accurate EMR documentation

What We’re Looking For

• Active Virginia PT license
• Home health experience preferred, but not required
• Strong clinical and communication skills
• Ability to work independently
• Reliable transportation
• Background check required
