Duties

• Assess patients’ physical conditions and develop tailored treatment plans that align with their individual needs and goals
• Implement therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and mobility training to facilitate recovery
• Collaborate closely with healthcare professionals including physicians, nurses, and occupational therapists to coordinate patient care and discharge planning
• Monitor patient progress regularly and adjust treatment strategies accordingly to optimize outcomes
• Maintain accurate clinical documentation in accordance with healthcare standards and electronic health records (EHR) management protocols
• Educate patients and their families on exercises, safety measures, and home modifications to support ongoing rehabilitation at home
• Conduct comprehensive discharge planning to ensure seamless transition from active therapy to community or home-based care

Skills

• Proven experience practicing in a physical therapy role within home health or outpatient rehabilitation settings
• Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology
• Ability to develop effective treatment plans based on thorough assessments and patient needs
• Excellent client communication skills to motivate patients and educate families effectively
• Familiarity with clinical documentation standards and EMR (Electronic Medical Records) systems for accurate record-keeping
• Experience working with geriatric patients and managing complex care plans in community or home environments
• Acute care experience in hospital or inpatient settings is highly valued for comprehensive patient assessment skills
• Knowledge of outpatient clinic procedures, outpatient rehabilitation center practices, and inpatient hospital protocols enhances service delivery excellence
