Job Summary

As a Home Health Physical Therapist, you’ll help patients build strength, mobility, and independence where they live. You’ll assess each patient’s needs, develop individualized treatment plans, and coordinate care with healthcare professionals and caregivers. Your work may support people recovering from illness or injury, managing chronic conditions, or navigating age-related changes.

Responsibilities

• Evaluate patient function and apply knowledge of anatomy, physiology, medical terminology, and physical therapy to guide care.
• Develop and update care plans and treatment plans; provide patient rehabilitation, including appropriate techniques such as trigger point therapy.
• Deliver home health and community-based patient care, including work with geriatric patients; adapt care for orthopedic, neurological, or pediatric needs as appropriate.
• Communicate with clients and caregivers, collaborate with healthcare professionals, and contribute to discharge planning.
• Complete physical therapy documentation in electronic health records (EHR) or electronic medical record (EMR) systems, following clinical documentation standards.

Experience

• Experience practicing in a physical therapy role and knowledge of physical therapy documentation are required.
• Home health or home and community care experience is valuable.
• Acute care, inpatient, hospital, clinic, outpatient, outpatient clinic, or outpatient rehabilitation center experience is preferred.
• Experience with geriatrics, orthopedics, neurology, or pediatrics is helpful.
• Familiarity with EHR/EMR management and accurate clinical records is preferred.

Pay: $90.00 - $100.00 per hour

Expected hours: 20.0 – 40.0 per week
