Job Overview
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Duties
- Develop and implement individualized treatment plans based on thorough assessments of patient needs, physiology knowledge, and medical history
- Conduct patient evaluations across outpatient clinics, home settings, and community care environments
- Perform therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and mobility training to promote recovery
- Manage clinical documentation standards by accurately recording treatment progress, discharge planning details, and therapy outcomes using electronic health records (EHR) and electronic medical records (EMR) systems
- Collaborate with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate comprehensive care strategies
- Assist in discharge planning by evaluating patient progress and recommending appropriate follow-up care or home & community care services
- Educate patients and their families on physiotherapy techniques, injury prevention strategies, and lifestyle modifications to support long-term health goals
Experience
- Proven practicing experience in a physical therapy role within outpatient clinics, school settings, or home health environments
- Demonstrated expertise in outpatient rehabilitation center experience or outpatient experience with a focus on orthopedics or neurology cases
- Prior work with geriatric patients or pediatrics is highly preferred; familiarity with working with diverse age groups enhances patient care quality
- Strong knowledge of anatomy and physiology principles relevant to physiotherapy practice
- Experience managing medical terminology accurately while adhering to clinical documentation standards
- Skilled in discharge planning processes for various patient populations including those requiring post-surgical or chronic condition management
- Familiarity with EMR systems such as hospital-based EHR platforms or outpatient clinic software is essential for efficient documentation and patient tracking
