Post Rehabilitation Services is hiring a Physical Therapist at Civita Care Center at Cheshire. You'll help residents regain mobility after illness or injury and address ongoing physical challenges through care tailored to their daily needs.

**What You'll Do**

• Evaluate residents' movement and establish functional treatment goals
• Provide therapy to improve strength, endurance, balance, and walking
• Address transfer safety and recommend appropriate mobility devices
• Reassess residents and update care plans as their abilities change
• Document outcomes and coordinate recommendations with nursing and the rehab team
