**Your Role**
- Perform in\-home PT evaluations
- Create and adjust personalized treatment plans
- Deliver skilled therapy to improve mobility, balance, and strength
- Educate patients and caregivers on exercises and safety
- Document progress and collaborate with PTAs and the care team
**What You Bring**
- Current **Massachusetts Physical Therapist (PT) license**
- At least **1 year of physical therapy experience**; home health experience preferred
- Strong clinical and communication skills
- Compassionate, patient\-centered approach
- Ability to work independently while collaborating effectively with a care team
- Successful completion of required background check and onboarding requirements
