Physician Services Group (PSG) is expanding our Post-Acute Care team and is seeking a Nurse Practitioner/ Physician's Assistant to support Skilled Nursing and Post-Acute facilities in the Georgetown, South Carolina area.

This position is ideal for an NP seeking work-life balance, schedule consistency, and clinical autonomy while building long-term patient relationships.

Schedule &

Work Environment

• Part-Time | 3 days per week (Monday–Friday)
• Typical hours: 8:00 AM – 4:00 PM
• On-site at Skilled Nursing / Post-Acute facilities
• No nights, weekends, or on-call

Key Responsibilities

• Evaluate and manage new admissions, follow-ups, and long-term care patients
• Provide chronic disease management and preventative care
• Collaborate with attending physicians and facility care teams
• Maintain accurate and timely clinical documentation
• Serve as PSG’s on-site clinical representative

Qualifications

• Active South Carolina Nurse Practitioner license
• Board Certified (ANCC, AANP, or equivalent)
• Strong clinical judgment and documentation skills
• Experience in post-acute, hospital, SNF, or outpatient settings preferred
• Interest in geriatrics and continuity of care strongly valued

Job Type: Part-time

Pay: $55.00 - $65.00 per hour
