Nurse Practitioner or Physician Assistant

Position Summary

We are seeking a compassionate, skilled, and team-oriented Nurse Practitioner or Physician Assistant to join our busy outpatient internal medicine clinic. The ideal candidate will provide comprehensive, patient-centered care for adults and seniors, focusing on preventive medicine, chronic disease management, and acute care within a collaborative medical team.

Key Responsibilities

• Provide direct patient care for adult and geriatric populations in an outpatient setting.
• Conduct comprehensive histories, physical exams, and develop appropriate diagnostic and therapeutic plans.
• Manage chronic medical conditions such as hypertension, diabetes, hyperlipidemia, and COPD in alignment with evidence-based guidelines.
• Order and interpret diagnostic tests, prescribe medications, and coordinate care with specialists as needed.
• Promote wellness and preventive care through counseling, education, and screenings.
• Document encounters thoroughly and accurately in the electronic health record (EHR).
• Participate in quality improvement initiatives and contribute to a positive, team-based work culture.
• Collaborate closely with physicians, nurses, and medical assistants to optimize patient outcomes and workflow efficiency.
