Salary Compensation:

$105,000 – $115,000 per year, with benefits.

Job Summary

We are seeking a compassionate, skilled, and motivated full-time Physical Therapist (PT) to join our home health team. This field-based position provides physical therapy evaluations, re-evaluations, and skilled treatment to patients in their homes, including clinical oversight of Physical Therapist Assistants (PTAs) in accordance with state practice guidelines.

The successful candidate must be available to work Tuesday through Saturday. The ideal candidate is patient-focused, organized, and passionate about helping individuals improve mobility, function, safety, and independence within their home environment.

Responsibilities:

  • Conduct comprehensive initial evaluations, re-evaluations, and discharge assessments for home health patients.
  • Develop and implement individualized plans of care in collaboration with the physician and interdisciplinary team, focused on improving strength, mobility, balance, endurance, and functional independence.
  • Monitor patient progress and modify the plan of care as clinically indicated.
  • Educate patients, family members, and caregivers regarding therapeutic exercises, safety practices, and home programs.
  • Provide clinical oversight and supervision of Physical Therapist Assistants (PTAs) in accordance with state practice act requirements.
  • Accurately document evaluations, patient visits, treatment interventions, progress, and outcomes in the electronic medical record.
  • Maintain timely completion of all required documentation and compliance standards.
  • Collaborate with interdisciplinary team members to coordinate patient care.
  • Adhere to infection control, patient safety, and professional practice standards.
  • Travel throughout designated service areas to provide in-home patient care.

Required Experience:

  • Graduate of an accredited Physical Therapist program.
  • Current and unrestricted Physical Therapist license in the state of practice.
  • Current CPR certification.
  • Valid driver’s license, automobile insurance, and reliable transportation.
  • Strong clinical, communication, and organizational skills.
  • Ability to work independently in a field-based environment.
  • Availability to work Tuesday through Saturday.

Preferred Experience:

  • Minimum of 3 years of home health experience, including performing evaluations and developing plans of care.
  • Experience utilizing electronic medical record systems.
  • Demonstrated ability to manage a patient schedule efficiently.
  • Strong knowledge of safety, fall prevention, and functional mobility interventions in the home setting.

Work Environment

Full-time, field-based home health position requiring travel throughout designated service areas. Regular schedule is Tuesday through Saturday, with independent patient visits and interdisciplinary team collaboration.
