Physical Therapist (PT) Home Health - Fort Lauderdale

Job Description

Pennie’s Home Health Care is seeking a compassionate, dependable, and experienced Licensed Physical Therapist (PT) to join our growing home health team.

The ideal candidate is dedicated to providing high-quality, patient-centered care while helping individuals improve mobility, restore independence, and enhance their overall quality of life in the comfort of their homes.

We offer flexible scheduling, competitive per-visit compensation, and opportunities for both W-2 employment and 1099 independent contracting, subject to applicable classification requirements.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations and assessments.
• Develop individualized treatment plans based on patient needs and physician orders.
• Provide skilled physical therapy services in patients' homes.
• Assist patients with improving strength, balance, coordination, mobility, and functional independence.
• Educate patients and caregivers on home exercise programs, fall prevention, and safety techniques.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals.
• Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
• Participate in care coordination and discharge planning.

Qualifications

• Active, unrestricted Physical Therapist license in the State of Florida.
• Graduate of an accredited physical therapy program.
• Current CPR/BLS certification.
• Previous home health experience preferred.
• Strong communication, organizational, and documentation skills.
• Ability to work independently and manage assigned patients.
• Reliable transportation, valid driver's license, and appropriate automobile insurance.
• Ability to travel to patients' homes throughout assigned service areas.
