Responsibilities

• Complete home visits including comprehensive assessments, reassessments, and discharge planning
• Develop and implement individualized patient care plans
• Provide patient and family education and support
• Collaborate with the interdisciplinary team to ensure high-quality outcomes

Qualifications

• Current RN license in the State of Ohio
• Valid driver's license and reliable transportation
• Strong clinical judgment, communication skills, and reliability
• Commitment to ongoing training, CEUs, and required certifications

Pay: $40.00 - $80.00 per hour
