Part-Time Hospice Start of Care Nurse (RN/BSN/MSN/NP)

ABOUT US

Parrish Palliative and Hospice Care of Ohio, LLC is a Joint Commission-accredited, nurse-owned Medicare hospice specializing in Alzheimer's and dementia care. We serve Lucas, Wood, Ottawa, and Fulton counties from our Sylvania Township office.

POSITION SUMMARY

We are seeking an experienced part-time Start of Care Nurse to complete hospice admissions in the home, facility, and community setting.

RESPONSIBILITIES

• Complete the initial and comprehensive assessment at start of care within required timeframes.
• Obtain and witness all admission consents, election statement, HIPAA acknowledgment, and advance directive documentation.
• Confirm and document hospice eligibility using applicable LCD criteria, FAST staging where appropriate, and general decline indicators, with related diagnoses documented as comorbidity support.
• Establish the initial plan of care and coordinate with the IDG, attending physician, and certifying physician.
• Complete medication reconciliation, DME and supply orders, and initial patient and family education.
• Submit all documentation same day or within agency timeframes in the electronic medical record.
• Communicate admission status promptly to the clinical supervisor and office.

REQUIREMENTS

• Active Ohio RN license in good standing.
• BSN, MSN, or Nurse Practitioner credential required.
• Hospice or home care experience required - no exceptions.
• Working knowledge of Medicare hospice Conditions of Participation, eligibility documentation, and LCD criteria.
• Strong assessment and documentation skills.
• Dependable transportation, valid driver's license, and current auto insurance.
• Current BLS certification.
• Comfortable working independently in the field.

Pay: $100.00 per hour

Expected hours: 5.0 – 10.0 per week
