Type of Employment: Full-time | Schedule: Monday-Friday (No Weekends)

What You'll Do

• Evaluations & Care Planning
• Provide ongoing Physical Therapy visits
• OASIS Documentation
• Communication with medical teams and families

What We're Looking For

• Active Pennsylvania physical therapist license required
• Experience in home health or short-term rehabilitation is preferred but not required
• Strong communication and problem-solving skills
• Ability to work independently and as part of a team
• Passion for helping seniors achieve their highest level of function
