About this role
Nurse Practitioner, Endocrinology and Metabolic Health | Upstate and downstate NY roles
Who is OwnaHealth?
OwnaHealth is a physician-led virtual clinic that reverses Type 2 diabetes and obesity with food, not more prescriptions. Our nutrition-first, low-carb approach helps patients in underserved communities lower their A1c by more than 2 points, and many come off insulin entirely.
We're a mission-driven health technology startup, and we're growing fast. We've signed several new contracts with community health centers and health systems across New York, with more launches coming in early 2027. We need great clinicians to help us scale.
Why join OwnaHealth?
- Reverse T2 diabetes, don't just manage it: Treat the cause with food as medicine, and watch patients; numbers move.
- Practice deprescribing: Take patients off insulin and the stack of medications they've carried for years.
- Learn from a world-class endocrinologist: Work directly with Dr. Mariela Glandt, our co-founder and CEO, a Harvard- and Columbia-trained endocrinologist who has helped many thousands of patients reverse diabetes.
- Time to actually know your patients: A reasonable caseload means real, long-term relationships with your patients.
- A team behind you: Care coordinators, coaches, dietitians and supervising endocrinology work alongside you.
- Technology that works for you: Our AI-powered platform takes on the busywork and monitors patients between visits, so you can spend your time on what you do best: caring for patients.
- Grow with us: You're at the core of what we do. As we scale, there's a path into clinical leadership.
What will you do?
- See patients by telehealth through Owna's AI-supported platform on a hybrid schedule, primarily on-site at a partner clinic in your region. With 50+ locations in both upstate and downstate New York, you'll work in your own region, with minimal local travel across the state.
- Manage a reasonable caseload of complex patients with uncontrolled Type 2 diabetes, obesity and related conditions.
- Lead medication management and deprescribing, including insulin, oral agents and antihypertensives.
- Use labs, CGM and remote monitoring data (glucose, weight, BP, ketones) to guide care.
- Build individualized low-carb and ketogenic treatment plans with the care team.
- Use motivational interviewing to support lasting behavior change, and keep documentation current.
What do you need to bring to the table?
- Active New York State NP license in good standing, with 3+ years of NP experience.
- Spanish fluency
- Experience managing Type 2 diabetes, obesity and cardiometabolic conditions.
- Genuine interest in low-carb nutrition, lifestyle medicine and deprescribing.
- Commitment to underserved, high-risk patients and a warm, non-judgmental approach.
- Comfort with EHRs and digital health tools (eClinicalWorks a plus).
- Ability to work full-time on a hybrid schedule, primarily on-site at a partner clinic in your region.
Compensation
- $80-85 per hour
- Training and non-clinical hours reimbursed
