Home Health Registered Nurse

Job Overview

As a Home Health Registered Nurse, you’ll bring skilled, compassionate care directly to patients in their homes. You’ll assess health needs, deliver treatments, coordinate care, and help patients and families understand each step of the plan. Your work supports safe recovery, greater confidence, and continuity of care outside the hospital.

Duties

• Assess patients, monitor vital signs, review symptoms, and document changes in electronic health records (EHR) while following Health Insurance Portability and Accountability Act (HIPAA) privacy requirements.
• Provide nursing care such as medication administration, intravenous (IV) infusion, catheter care, wound and ostomy care, and infection control, within your scope of practice.
• Coordinate with patients, families, providers, and care teams on case management, referrals, and discharge planning.
• Teach patients and caregivers about medications, nutrition, symptom monitoring, and home care routines.

Skills

• Required: Current registered nurse licensure and nursing experience relevant to home care; strong patient assessment, communication, and medical documentation skills.
• Required: Knowledge of medication administration, aseptic technique, patient safety, and electronic medical record (EMR) or EHR systems; familiarity with medical terminology, anatomy, and physiology.
• Preferred: Home care, hospice and palliative medicine, senior care, geriatrics, long-term care, assisted living, memory care, dementia care, or nursing home experience.
• Preferred: Experience with IV insertion, venipuncture, phlebotomy, injections, tube feeding, ventilator care, triage, or acute pain management.
• Preferred: Epic, Cerner, eClinicalWorks, basic life support, ICD-10 coding, case management, or experience supporting people with disabilities.

Pay: $50.00 - $100.00 per hour

Expected hours: 5.0 – 25.0 per week
