Ohman Family Living is hiring a PRN Physical Therapist (PT) for our home health team in Northeast, Ohio. Pay: $60–$150 per visit, depending on visit type

Physical Therapist Responsibilities:

• Conduct initial evaluations and develop individualized treatment plans based on patient needs and goals
• Provide skilled physical therapy treatment to improve strength, mobility, balance, and coordination
• Supervise and delegate care to Physical Therapist Assistants (PTAs) as appropriate
• Educate patients and caregivers on safety techniques, home exercise programs, and use of assistive devices
• Monitor patient progress and adjust treatment plans as needed
• Complete clinical documentation in compliance with home health and Medicare regulations

PT Qualifications:

• Active Physical Therapist (PT) license in the state of Ohio
• Home health or rehabilitation experience preferred
• Excellent verbal and written communication skills
• Strong critical thinking and clinical judgment
• Compassion, patience, and a positive attitude
• Valid driver's license with reliable vehicle and car insurance
• Ability to work independently and travel to patient homes
• Must pass a background check
