About Us
Ohman Family Living has been caring for families in Geauga County since 1965. We’re still family-owned, and that shows up in how we treat our patients and our team. Our home health agency, The Hills at Home, brings skilled nursing and therapy into patients’ homes so they can recover and stay independent where they’re most comfortable.
We’re looking for an experienced RN Case Manager who knows OASIS inside and out and wants to work somewhere that values clinical judgment over checking boxes.
What You’ll Do
- Manage a caseload of home health patients
- Complete OASIS assessments at start of care, resumption of care, recertification, transfer, and discharge
- Develop and update individualized plans of care in coordination with physicians, therapists, and the care team
- Provide skilled nursing visits, including wound care, medication management, disease management, and patient/caregiver education
- Coordinate with physicians and specialists on orders, changes in condition, and care transitions
- Supervise LPNs and home health aides as needed
- Document accurately and on time to support quality outcomes and compliance with Medicare and state regulations
- Act as the main point of contact for your patients and their families
What You’ll Bring
- Active Ohio RN license (or compact license with Ohio privileges)
- Minimum 1 year of home health experience with OASIS assessments and documentation
- Strong understanding of Medicare home health guidelines and PDGM
- Current CPR/BLS certification
- Valid driver’s license, reliable transportation, and auto insurance
- Comfortable with EMR documentation [add system name if you want to call it out]
- Organized, self-directed, and good at building trust with patients and families
Preferred
- BSN
- COS-C certification
- Wound care experience
Why Ohman
- Competitive pay and mileage reimbursement
- Health, dental, and vision insurance
- Paid time off
- 401(k) [with match, confirm]
- Manageable caseloads and a supportive clinical team
- Leadership you’ll actually know by name
- 60 years of stability in a family-owned organization
Benefits:
- 401(k) matching
- Dental insurance
- Health insurance
- Paid time off
- Vision insurance
Work Location: In person
