Novant Health Cotswold Medical Clinic is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to join our team. This role will have heavy patient triage management. This role collaborates closely with physicians and interdisciplinary team members to assess, plan, implement, and evaluate individualized plans of care that promote optimal health outcomes and exceptional patient experiences.

What We're Looking For:

• Education: 4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required. If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
• Experience: One year relevant experience, preferred.
• Licensure/Certification/Registration: Current RN license in appropriate state Required. ACLS, PALS, BAT and DOT, preferred.

What You'll Do:

• Conduct Medicare Annual Wellness visits and Chronic Care Management (6-8 patients a day)
• Conducting patient outreach for patients that are due for annual metrics
• Assist providers in merging gaps in providing patients with the necessary resources for their community needs.
