**Job Summary:**
We are seeking a compassionate and skilled Physical Therapist to join our team at Northwood Skilled Nursing and Rehabilitation. As a Physical Therapist at Northwood, you will have the opportunity to work with a diverse population of patients and provide individualized rehabilitation services to help them achieve their maximum potential.
**Responsibilities:**
- Evaluate and treat patients with a wide range of medical conditions and diagnoses
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Provide one\-on\-one therapy sessions, including physical therapy, occupational therapy, and speech therapy
- Collaborate with other members of the interdisciplinary care team to ensure comprehensive care
- Maintain accurate and timely records of patient treatment and progress
- Participate in patient education and training to promote independence and self\-care
**Requirements:**
- Bachelor's or Master's degree in Physical Therapy from an accredited program
- Current licensure as a Physical Therapist in the state of Ohio
