About the Role

Home Care Services of Northwest Indiana is seeking a part-time Physical Therapist who values independence and wants more control over their schedule. In this role, you'll manage your own caseload, plan your own day, and provide one-on-one care to patients in the comfort of their homes across the Michigan City and La Porte area.

Whether you're looking to supplement a full-time position, transition away from facility-based therapy, or build a schedule around family, this role offers the flexibility to practice on your terms.

Key Responsibilities

Patient Evaluation & Care Planning

• Complete initial evaluations, re-evaluations, and discharge assessments in the home setting
• Develop individualized plans of care with measurable, functional goals
• Complete OASIS assessments as required (Start of Care, Resumption of Care, Discharge)

Treatment & Patient Education

• Provide skilled interventions including gait training, therapeutic exercise, balance and fall prevention, transfer training, and DME recommendations
• Educate patients and caregivers on home exercise programs, safety, and injury prevention
• Assess home environments and recommend modifications to support safe, independent living

Collaboration & Documentation

• Coordinate care with nursing, OT, SLP, and other members of the care team
• Supervise PTAs as applicable
• Complete timely, accurate documentation in the EMR in accordance with Medicare and agency guidelines

Qualifications

Required

• Degree from an accredited Physical Therapy program
• Current Indiana Physical Therapist license (or PT Compact privilege to practice in Indiana)
• Current CPR/BLS certification
• Valid driver's license, reliable transportation, and current auto insurance

Preferred

• 1+ year of home health experience
• Familiarity with OASIS documentation and Medicare guidelines
• Experience with home health EMR systems

Compensation & Benefits

Pay: $45.00 - $65.00 per hour

Expected hours: 19.0 – 29.0 per week
