Responsibilities:

• Treat geriatric and/or pediatric patients in their homes within East Brooklyn County.
• Create and execute personalized care plans that debunk the myth that home care is "low skill."
• Submit clinical documentation in a timely manner.
• Communicate openly with the leadership team—we want to hear your ideas and feedback.

Qualifications:

• License: Valid NY State Physical Therapy License (Required).
• Education: Bachelor’s or Doctorate degree in Physical Therapy.
• Experience: New Grads Welcome! We invest in training the right people.
• Transport: Ability to travel to patient homes in East Brooklyn County.

Pay: $90,000.00 - $110,000.00 per year
