Overview
We are seeking a dedicated and compassionate Physical Therapist to join our rehab team in a skilled nursing facility setting. The ideal candidate will be responsible for assessing, planning, and implementing therapeutic interventions to help patients improve their ability to perform daily activities.
This role requires a strong understanding of patient care and the ability to work collaboratively with other healthcare professionals to enhance patient outcomes.
Duties
- Conduct comprehensive assessments of patients' physical, emotional, and social needs.
- Develop individualized treatment plans based on assessment findings and patient goals.
- Implement therapeutic interventions aimed at improving patients' functional abilities and quality of life.
- Collaborate with multidisciplinary teams, including physicians, nurses, and speech therapists, to ensure comprehensive patient care.
- Document patient progress and adjust treatment plans as necessary based on ongoing evaluations.
- Perform progress notes, recertifications, and discharge summaries as needed.
- Maintain knowledge of medical terminology, case management practices, and discharge planning protocols.
Experience
Candidates should possess the following skills and experience:
- Knowledge of physiology and medical terminology relevant to physical therapy practices.
- Familiarity with working with the geriatric population and understanding their unique needs.
- Experience in skilled nursing facility settings is a plus but not mandatory.
Schedule
This position is required to provide monthly availability and will be scheduled on an as-needed basis for weekend coverage. When scheduled, this position allows for flexible working hours to complete patient evaluations and treatments anytime between 7:30AM and 8:00PM (PST).
Qualifications
This position is required to meet the following:
- Doctorate of Physical Therapy or equivalent required.
- Must possess an active license from the California Board of Physical Therapy
- Must possess active CPR certification
