Home Health Physical Therapist – Full-Time

Nayar Health Care is currently seeking a Full-Time Home Health Physical Therapist to join our growing team.

This opportunity is ideal for a Physical Therapist seeking schedule flexibility, meaningful one-on-one patient care, and the support of a respected home health organization. What You’ll Do

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide skilled physical therapy services to restore function, reduce pain, improve mobility, and prevent disability
• Review patients’ medical histories and perform initial assessments and reassessments
• Establish measurable goals and update treatment plans based on patient progress
• Educate patients, caregivers, and families regarding exercises, safety, mobility, and treatment goals
• Collaborate with physicians and members of the interdisciplinary care team
• Complete timely and accurate clinical documentation in accordance with agency, state, federal, and payer requirements
• Develop effective discharge plans that promote patient safety and independence

Requirements

• Current and unrestricted Physical Therapist license
• Valid driver’s license, reliable transportation, and current automobile insurance
• Ability and willingness to travel to patients’ homes throughout the assigned territory
• Ability to attend occasional meetings or pick up supplies at the office as needed
• Strong communication, organization, and time-management skills
• Ability to work independently while remaining connected to the interdisciplinary care team

Preferred Experience and Qualifications

• Graduate of an accredited Physical Therapy program
• Current CPR certification
• Minimum of one year of experience as a Physical Therapist
• Previous home health experience preferred
• Experience providing care to adult and geriatric patients preferred
