Job title: Per Diem Physical Therapist (Home / Community): Bayonne,NJ

Mobile PhysioCare is hiring a per diem Physical Therapist for one-on-one outpatient visits in patients' homes, assisted living facilities, and group homes.

This is outpatient physical therapy in the home, not home health. There is no OASIS documentation and no homebound requirement.

Towns covered

Secaucus, Bayonne, Jersey City, North Bergen, West New York, Cliffside Park, Little Ferry, Englewood, Harrington Park, and Cresskill. Pick the towns you can realistically cover.

Caseload includes Medicare Part B, commercial PPO, NJ DDD (developmental disability), and cash-pay patients.

Responsibilities

• Evaluate and treat adults and older adults using the real home setting (stairs, tub, kitchen, sidewalks)
• Provide 1:1 treatment in private homes, assisted living, and group homes
• Write timely outpatient (Part B) documentation
• Coordinate with families, physicians, and the therapy team
• Supervise PTAs on shared patients when assigned

Requirements

• Current New Jersey PT license (or compact privilege to practice in NJ)
• Doctor of Physical Therapy or equivalent entry-level PT degree
• Valid driver's license, reliable vehicle, and auto insurance
• Ability to treat independently in the community and safely transfer or guard patients

Preferred

• Geriatric, neurologic, or developmental-disability experience
• Spanish or Korean
• Ability to cover more than one listed town

Pay and benefits

• $65 - $75 per hour, W-2 per diem (1099 option available at a higher contracted rate)
• W-2: paid biweekly, with a $500 - $1,000 visit advance available after your first completed week (deducted from your second paycheck)
• W-2: mileage reimbursed at the IRS standard rate
• W-2: company-paid professional liability insurance
• 1099: paid weekly after signed notes
