Duties

• Conduct thorough patient assessments to determine health status and care needs using advanced medical software and electronic health records (EHR) systems.
• Develop and implement individualized care plans aligned with health regulation policies and best practices in disease prevention and chronic disease management.
• Administer medications, treatments, and emergency care following established protocols such as first aid, CPR, and basic life support (BLS).
• Monitor patient progress, document medical records accurately, and update electronic health records (EHR) to ensure compliance with HIPAA regulations and medical documentation standards.
• Provide patient education on physiology, anatomy, disease processes, medication management, and self-care strategies to promote health literacy.
• Coordinate with physicians, therapists, and other healthcare professionals through triage and communication to optimize patient outcomes.
• Handle confidential information with integrity while managing medical records securely and adhering to privacy laws such as HIPAA.
• Respond promptly to urgent situations requiring acute or emergency care, demonstrating proficiency in emergency response procedures.
• Support families and caregivers by offering guidance on home safety measures and disease prevention techniques.

Experience

• Proven experience in nursing with a focus on home health or outpatient care environments.
• Demonstrated ability to utilize medical software, EMR/EHR systems, and medical terminology effectively.
• Hands-on experience in emergency care procedures including CPR, first aid, and BLS certification.
• Knowledge of health regulation policies related to home healthcare services and compliance standards such as HIPAA.
• Strong understanding of physiology, anatomy, chronic disease management, and acute care practices.
• Previous nursing leadership experience is a plus for mentoring team members or supervising new staff.
• Excellent communication skills for triage assessments, patient education sessions, and interdisciplinary collaboration.

Pay: $55.00 - $60.00 per hour
