Midwest Urgent Care is seeking a dedicated Primary Care Nurse Practitioner (FNP) or Physician Assistant (PA-C) to join our team in a part-time capacity. While this role is primarily focused on building a "medical home" for our patients through routine health maintenance and chronic disease management, you will also evaluate and manage acute, walk-in urgent care patients as clinic needs dictate.

This position provides a dynamic clinical environment that blends long-term patient relationships with normal-paced acute interventions.

Key Responsibilities

• Primary Care & Chronic Care Management (CCM): Act as the primary clinician for scheduled primary care visits, managing individualized, evidence-based care plans for complex, long-term conditions (e.g., Diabetes, Hypertension, COPD, Asthma, CKD).
• Urgent Care & Acute Coverage: Provide prompt evaluation, triage, and management for walk-in urgent care patients presenting with acute illnesses and minor injuries.
• Health Maintenance & Prevention: Perform annual wellness exams, DOT, routine physicals, and preventive health screenings to promote community wellness and disease prevention.
• Diagnostics: Order and interpret diagnostic tests, including point-of-care laboratory tests and diagnostic imaging (X-rays, CT, MRI), for both chronic monitoring and acute assessment.
• Clinical Procedures: Perform routine outpatient and urgent care procedures, including simple laceration repairs (suturing/stapling), abscess incision and drainage, basic wound care, and IV infusion management.
• Behavioral & Preventive Screenings: Evaluate physical and mental health intersections, including standardized screenings for depression, anxiety, and lifestyle-related risks.
• Documentation & Care Coordination: Maintain accurate, timely documentation in the EHR utilizing proper CPT/ICD-10 coding for both E/M primary care and urgent care visits. Coordinate specialist referrals and follow-up care as needed.

Required Skills & Qualifications

• Education: Graduate of an accredited Nurse Practitioner (MSN/DNP) or Physician Assistant (MSPAS/MHS) program.
• Licensure & Certifications: Active, unrestricted APRN or PA license to practice in Minnesota, current DEA registration, and BLS certification (DOT certification is a plus).
• Experience: Minimum of 1 years of clinical experience in Family Medicine or Internal Medicine, with comfort or previous exposure to urgent care/ambulatory acute care settings.
• Technical Savvy: High proficiency with electronic health record (EHR) documentation and digital clinical workflows.
• Communication: Strong interpersonal and diagnostic skills, with an emphasis on patient-centered communication and efficient clinical decision-making.
