We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care, conduct assessments, and develop treatment plans in collaboration with physicians and other healthcare professionals. This role is essential for delivering comprehensive care to patients, particularly those with complex health needs.

Responsibilities

• Conduct thorough patient assessments and evaluations to determine health status.
• Develop and implement individualized treatment plans based on patient needs including Diagnoses, Care plans, lab orders, Imaging orders, and Referrals.
• Provide care in specialized areas, including infectious disease management.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Educate patients and families about health management, including at-home care.
• Maintain accurate medical records and documentation in compliance with healthcare regulations.
• Stay current with advancements in medical practices and participate in ongoing professional development.

Skills

• Strong knowledge of anatomy and human systems relevant to patient care.
• Experience in medical- nursing practices.
• Familiarity with ICD-9 coding for accurate diagnosis documentation.
• Ability to work effectively with individuals with developmental disabilities.
• Excellent communication skills for effective interaction with patients, families, and healthcare teams.
• Proficient in performing clinical procedures, including Diagnoses, Care plans, lab orders, Imaging orders, and Referrals.
