Temp - Registered Nurse - Home Health

NOW HIRING: HOME HEALTH REGISTERED NURSE (RN)* Schedule & Hours

• Monday–Friday.
• Day Shift: 8:00 AM – 5:00 PM.
• 40 hours guaranteed per week.

Responsibilities:* Perform comprehensive patient assessments and ongoing evaluations.

• Develop, implement, and update individualized plans of care.
• Complete OASIS assessments and required documentation.
• Coordinate patient care with physicians, CNAs, caregivers, and other healthcare professionals.
• Establish discharge plans and provide patient and family education.
• Ensure compliance with applicable state, federal, and agency regulations.
• Maintain patient privacy, confidentiality, and a safe care environment.

Requirements:* Home Health experience required.

• Active BLS certification or willingness to obtain.
• Valid driver's license and car insurance.
• Active Illinois nursing license required at submission.
• COVID-19 and flu vaccinations required; declinations will not be accepted.
• Strong Med-Surg experience with some Home Health experience may be considered.
• OASIS and Start of Care experience strongly preferred.
• Local candidates preferred.
