Job Overview

We are seeking a passionate and dedicated Occupational Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement personalized therapy programs to improve patients’ functional abilities across a diverse range of settings.

Responsibilities

  • Conduct comprehensive evaluations of patients’ physical, cognitive, and emotional functioning using physiology knowledge and medical terminology.
  • Develop and implement individualized care plans tailored to each patient’s unique needs, including assisting with activities of daily living (ADLs), mobility, and sensory integration.
  • Collaborate with multidisciplinary teams—including speech therapists, physicians, case managers, and educators—to coordinate holistic care strategies.
  • Support discharge planning by preparing patients for transition back home or to other care environments through tailored therapy goals and caregiver education.
  • Manage case documentation meticulously, including progress notes, treatment plans, and discharge summaries aligned with healthcare standards.
  • Engage in patient care activities that promote recovery and independence while adhering to safety protocols within post acute care or independent living settings when applicable.
  • Contribute to the development of care plans that incorporate occupational health principles for workplace injury prevention or ergonomic assessments.

Requirements

  • Valid licensure or certification as an Occupational Therapist in the relevant jurisdiction.
  • Proven experience working with people with disabilities across multiple settings such as geriatrics, outpatient clinics, inpatient units, skilled nursing facilities, or home health environments.
  • Strong knowledge of anatomy and physiology as well as medical terminology relevant to occupational therapy practice.
  • Experience assisting with ADLs and implementing discharge planning strategies effectively.
  • Excellent communication skills for collaborating within multidisciplinary teams and educating patients and caregivers.
  • Flexibility to adapt therapy approaches based on patient needs across inpatient, outpatient, or community settings.
