**Overview**
Join our dynamic healthcare team as a PRN (as\-needed) Physical Therapist dedicated to delivering exceptional patient care across diverse settings. In this role, you will utilize your expertise in physiotherapy to develop personalized treatment plans, assist patients in their rehabilitation journeys, and collaborate with multidisciplinary healthcare professionals.
Your energy and commitment will help improve mobility, reduce pain, and enhance quality of life for patients of all ages, including geriatrics, pediatrics, and those recovering from acute or chronic conditions. This flexible position offers the opportunity to make a meaningful impact while working in outpatient clinics, inpatient hospital units, home health environments, and outpatient rehabilitation centers.
**Responsibilities**
- Assess patient needs through comprehensive evaluations, including physiology and anatomy knowledge, to inform effective treatment strategies.
- Develop and implement tailored care plans focused on patient rehabilitation goals, discharge planning, and overall recovery progress.
- Deliver evidence\-based therapies such as manual therapy, trigger point therapy, and other modalities suited to individual patient requirements.
- Document all clinical interactions accurately following medical terminology standards and clinical documentation protocols within electronic health records (EHR) or electronic medical records (EMR) systems.
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care plans and optimize patient outcomes.
- Manage patient progress by adjusting treatment plans based on ongoing assessments and response to therapy.
- Support patients’ transition from inpatient to outpatient or home \& community care settings by providing education and discharge instructions.
**Requirements**
- Valid state licensure as a licensed Physical Therapist with practicing in a physical therapy role.
- Demonstrated experience in acute care, inpatient hospital settings, outpatient clinics, or home health environments.
- Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
- Proven ability to develop treatment plans that incorporate discharge planning and patient care goals effectively.
- Familiarity with outpatient rehabilitation center operations and outpatient clinic experience preferred.
- Experience working with geriatric patients and understanding of age\-specific care considerations.
- Proficiency in clinical documentation standards and electronic health records (EHR/EMR systems).
- Excellent client communication skills to educate patients about their treatment plans and foster engagement in their recovery process.
- Ability to work collaboratively with healthcare teams across inpatient, outpatient, hospital, and home \& community settings.
