Responsibilities

As a Home Health PT, you will:

  • Perform comprehensive physical therapy evaluations and reassessments, including OASIS assessments as assigned, and develop individualized plans of care with measurable functional goals.
  • Provide skilled therapy visits in patients' homes, assisted living, or other residential settings.
  • Assess mobility, strength, balance, gait, transfers, endurance, and home safety; identify fall risks and recommend home modifications.
  • Provide therapeutic exercise, gait and transfer training, balance training, neuromuscular re-education, and other skilled interventions per the plan of care.
  • Recommend and train patients on durable medical equipment and assistive devices (walkers, wheelchairs, hospital beds, etc.).
  • Educate patients and caregivers on home exercise programs, safe transfers, fall prevention, and body mechanics.
  • Supervise Physical Therapist Assistants (PTAs) as required and consistent with Ohio practice regulations.
  • Coordinate with the interdisciplinary team, including physicians, nurses, OTs, SLPs, social workers, and home health aides, to meet patients' needs.
  • Communicate changes in patient condition or function to physicians and the clinical team.
  • Complete accurate and timely clinical documentation in the agency's electronic medical record system, including documentation supporting medical necessity and skilled need; take part in care planning and case conferences.
  • Help patients safely stay at home whenever possible.
  • Maintain compliance with Medicare home health regulations, HIPAA, agency policies, and applicable state and federal regulations.

Qualifications

  • Degree in Physical Therapy (DPT or MPT) from an accredited program.
  • Current and unrestricted Ohio Physical Therapist license.
  • Current BLS/CPR certification.
  • Ideally 1 year of home health experience.
  • Experience in hospitals, skilled nursing, outpatient, rehabilitation, or other clinical settings is welcome.
  • Strong evaluation and clinical reasoning skills.
  • Excellent communication and patient/caregiver education skills.
  • Comfortable working independently in patients' homes.
  • Reliable transportation and ability to travel within the assigned Cleveland area territory.
  • Strong organizational and time-management skills.
  • Comfortable using electronic medical record systems and mobile technology.
