About this role
Senior Physician Assistant – Clinical Partner to the President
Dr. Mary Tilak & Associates | Northwest Indiana
Full-Time | Minimum 5 Years of Clinical Experience | Advanced Primary Care
An Exceptional Opportunity for an Experienced Physician Assistant
At Dr. Mary Tilak & Associates, we are making a deliberate investment in clinical excellence as we continue building a scalable, physician-led, team-delivered model of Advanced Primary Care.
We are seeking an accomplished Physician Assistant (PA-C) with at least five years of meaningful frontline clinical experience to serve as a Senior Clinical Partner to our President, Dr. Mary Tilak, MD, MBA.
This is a distinctive opportunity for a clinician who combines exceptional clinical judgment with strong communication, collaborative leadership, and the ability to navigate complex patient care.
The purpose of this position is twofold: to strengthen the clinical expertise, coordination, and consistency of care delivered across our teams, while extending the capacity of our physician leader to focus on healthcare innovation, strategic development, and relationships that advance our care model.
We are not simply looking to add another provider. We are looking for a trusted clinical partner who wants to make a broader impact on patient care.
Your Role
Working directly with Dr. Tilak, you will serve as a senior clinical resource for complex patient management, care coordination, clinical communication, and collaboration across our Advanced Practice Provider (APP) teams.
You will help ensure that clinical decisions, escalation pathways, and multidisciplinary care remain timely, thoughtful, evidence-based, and aligned with our standards of excellence.
This role combines hands-on clinical practice with a unique opportunity to contribute to the development of an innovative healthcare delivery model.
Key Responsibilities
1. Advanced Clinical Care and High-Risk Patient Management
- Provide high-level clinical assessment and management of patients with complex, multiple chronic conditions.
- Apply sophisticated diagnostic reasoning, evidence-based medicine, and guideline-directed medical therapy.
- Recognize early clinical deterioration, anticipate risks, and facilitate timely intervention.
- Collaborate closely with Dr. Tilak on complex clinical decisions, treatment plans, and multidisciplinary care.
- Coordinate care with specialists, hospitals, and tertiary academic centers when clinically appropriate.
2. Clinical Partnership and Communication
- Serve as a trusted clinical liaison between Dr. Tilak, APPs, and other clinical teams.
- Evaluate, prioritize, and resolve appropriate clinical questions and escalations within the PA's scope of practice.
- Ensure matters requiring physician involvement are communicated promptly with clear clinical context and recommendations.
- Facilitate effective communication with external specialists and healthcare partners.
- Streamline clinical information and coordination so that physician involvement is focused where it adds the greatest value.
3. APP Collaboration and Clinical Excellence
- Work collaboratively alongside our Nurse Practitioners, Physician Assistants, and Population Health Management teams to strengthen clinical consistency and coordination.
- Serve as an accessible clinical resource for complex cases, shared decision-making, and timely escalation.
- Reinforce evidence-based clinical standards, documentation quality, and appropriate escalation pathways.
- Facilitate clinical discussions, shared learning, and constructive feedback without creating unnecessary layers of hierarchy.
- Support the consistent application of our Advanced Primary Care model through professional collaboration, mutual respect, and shared accountability for patient outcomes.
4. Clinical Quality and Innovation
- Support risk-stratified management of chronic disease populations.
- Use clinical data, remote patient monitoring, and care management insights to guide proactive intervention.
- Identify opportunities to strengthen clinical workflows, patient safety, and care continuity.
- Participate in continuous clinical quality improvement and the evolution of technology-enabled care delivery.
- Contribute frontline clinical insights to the development of scalable, outcome-driven care models.
Clinical Coordination, Collaboration, and Accountability
This position carries clinical coordination responsibility, not automatic supervisory authority over other Advanced Practice Providers.
The Senior Clinical
Partner will work directly with Dr. Tilak to strengthen clinical decision-making, communication, care coordination, and the consistent application of evidence-based standards across our clinical teams.
The role is designed to complement and support the expertise of our APPs—not to create an additional hierarchical layer of clinical management.
Clinical influence will be established through professional credibility, sound judgment, effective communication, and the ability to facilitate collaboration around complex patient needs.
Formal operational management, staffing, scheduling, and administrative accountability remain within the organization's established management structure.
The Senior Clinical
Partner will report directly to the President for clinical integration responsibilities and collaborate closely with operational leadership to ensure that clinical priorities translate into consistent and effective care delivery.
Our expectation is leadership through clinical expertise and collaboration, with shared ownership of patient outcomes.
Qualifications
Required
- Graduate of an accredited Physician Assistant program.
- Current NCCPA certification and active or eligible Indiana licensure.
- Minimum five years of direct clinical experience, preferably in internal medicine, hospital medicine, complex primary care, or another clinically demanding setting.
- Demonstrated ability to manage patients with multiple chronic diseases and complex medical needs.
- Strong clinical reasoning, sound judgment, and an understanding of when physician collaboration is essential.
- Exceptional communication, organizational, and interpersonal skills.
- Commitment to evidence-based, patient-centered, collaborative care.
The Individual We Hope to Find
You are a clinician who:
- Enjoys the intellectual challenge of complex medicine.
- Is confident in clinical judgment while remaining humble and collaborative.
- Anticipates problems rather than simply responding to them.
- Communicates clearly, concisely, and with professional discretion.
- Can influence clinical excellence through expertise, trust, and collaboration rather than hierarchy.
- Values the integration of human clinical judgment with data, technology, and intelligent automation.
- Finds professional fulfillment in helping other clinicians and patients succeed.
- Wants to contribute to something larger than an individual clinical schedule.
Why This Position Is Different
For 25 years, Dr. Mary Tilak & Associates has remained committed to delivering thoughtful, comprehensive, relationship-centered medical care to patients across Northwest Indiana.
Today, we are intentionally evolving that foundation into an Advanced Primary Care ecosystem that combines experienced clinicians, population health management, technology, and meaningful specialist collaboration.
This new position reflects our belief that clinical excellence requires investment in people, judgment, collaboration, and systems—not simply increasing patient volume.
As Senior Clinical
Partner to the President, you will have the opportunity to work closely with an experienced physician leader, participate in complex medical decision-making, contribute to clinical team development, and help advance a model designed to improve patient outcomes while remaining operationally sustainable.
This is a role for someone who wants to practice medicine with purpose and help shape how care is delivered.
Compensation and Professional Growth
We offer competitive compensation based on experience, clinical expertise, and the scope of this specialized position, along with benefits and opportunities for professional growth.
The position is designed as a long-term investment in clinical excellence, with opportunities for expanded responsibility as our Advanced Primary Care model evolves.
