We are seeking a licensed Physical Therapist professional to join our team, dedicated to providing top\-notch care and support to our patients. As a Physical Therapist at Marlora Post Acute Rehabilitation, you will work with patients to guide them through personalized therapy and help them achieve their recovery goals.

**Responsibilities:**

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
• Monitor and document resident progress and adjust treatment plans as necessary
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents
• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs
• Maintain accurate and timely documentation of physical therapy services provided
• Participate in team meetings, in\-service education, and quality improvement initiatives
• Maintain current knowledge of physical therapy practice and relevant regulations

**Qualifications:**

• Current license to practice as a Physical Therapist in the state of California
• Knowledge of current physical therapy practice standards and regulations
