LTC Therapy Services is hiring a Physical Therapist for GlenRidge/Courtyard in Medford. You'll help residents rebuild mobility after a hospital stay and address ongoing movement challenges, with treatment grounded in their daily needs.

**What You'll Do**

• Evaluate functional mobility and identify each resident's treatment priorities
• Develop individualized plans for strength, balance, walking, and transfers
• Deliver therapy and adjust interventions based on progress and tolerance
• Guide residents and caregivers on safe movement and appropriate assistive devices
• Maintain accurate clinical records and coordinate with nursing and the rehab team
