LTC Therapy Services is seeking a Physical Therapist at Blackstone Valley Health and Rehabilitation. In this skilled nursing role, you'll help residents work toward meaningful milestones, from standing safely to walking with greater confidence.

**What You'll Do**

• Assess residents' physical abilities and establish measurable recovery goals
• Plan and provide treatment for mobility, strength, balance, and endurance
• Practice gait and transfers using appropriate assistive devices
• Reassess outcomes and modify care as residents' needs change
• Document treatment and share recommendations with nursing and fellow clinicians
