Physical Therapist PRN

Role Overview: The physical therapist assesses, plans, and implements rehabilitative programs to improve mobility, reduce pain, increase strength, and prevent deformities in patients with disease or injury. Services are delivered across the continuum with a focus on quality, safety, and regulatory compliance.

Key Responsibilities:

• Patient Assessment: Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies, involving patients and families.
• Treatment Planning and Care: Execute treatment plans, select appropriate activities, educate patients and families, and provide discharge instructions and referrals.
• Documentation: Record evaluations, treatment sessions, and discharge summaries following departmental protocols.
• Professional Conduct: Uphold ethical standards, maintain confidentiality, punctuality, dress code, and effective communication; participate in meetings and process improvements.
• Customer Service & Teamwork: Foster a positive environment, demonstrate respect, apply service recovery principles, and support team collaboration.
• Professional Development: Engage in ongoing education, complete required competencies, training, and licensure maintenance, and assist in onboarding new staff.

Qualifications & Skills:

• Masters in Physical Therapy required; Doctorate preferred.
• Licenses and certifications: PT license, BLS certification.
• Minimum one year of experience in rehab, LTACH, acute care, or SNF settings.
• Effective communication, customer service orientation, teamwork, and basic computer skills.
