Role Overview: The Physical Therapist provides and supervises rehabilitative care to patients in their home setting, ensuring age-appropriate interventions that address physical, emotional, developmental, psychosocial, and educational needs. The role involves coordinating with multidisciplinary teams, developing and revising care plans, and managing patient progress from admission to discharge.
Key Responsibilities:
- Assess, plan, implement, and adjust patient care based on physician orders and patient needs.
- Supervise PTAs and Home Health Aides, ensuring quality care delivery throughout the episode of care.
- Communicate patient status updates to physicians and team members, maintaining documentation accuracy.
- Educate patients and caregivers to promote independence and self-care during recovery.
- Participate in staff training, continuing education, and quality improvement initiatives.
Qualifications & Skills:
- Graduate of an accredited physical therapy program with a valid license.
- Minimum of 1 year of full-time physical therapy experience; home health experience preferred.
- Current BLS certification, valid driver’s license, and private vehicle insurance.
- Strong assessment, rehabilitation, communication, and organizational skills.
- Proficiency with home health software, understanding of regulatory guidelines, and OASIS knowledge is preferred.
Additional Duties
Provide comprehensive patient care, participate in performance improvement activities, and support agency goals. Must have a valid driver’s license and be willing to participate in weekend and call rotations.
