Role Overview: The Physical Therapist provides primary care to approximately 25-35 patients in their homes within a specified geographic area, focusing on rehabilitative interventions tailored to patients’ physical, emotional, and developmental needs.
They supervise PTAs and Home Health Aides, coordinate care across disciplines, and ensure goals are achieved through effective communication with physicians, patients, families, and team members.
Key Responsibilities:
- Assess, develop, and manage individualized care plans aligned with physician orders.
- Perform routine and complex therapy procedures, adjusting frequency based on patient status.
- Supervise and review care provided by support staff, ensuring quality and compliance.
- Educate patients and caregivers to promote self-care and independence.
- Participate in team conferences, document accurately in EMR, and adhere to regulatory guidelines.
- Engage in performance improvement activities and contribute to department goals.
Qualifications & Skills:
- Graduate of an accredited physical therapy program.
- Valid Physical Therapist license, BLS certification, and Georgia driver’s license with proof of insurance.
- Minimum of 1 year of full-time PT experience, preferably in home health.
- Strong assessment, rehabilitation, communication, and organizational skills.
- Proficient in home health operations, documentation, and regulatory compliance; OASIS knowledge preferred.
