Physical Therapist

Role Overview: The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary environment. They are responsible for ensuring quality care, contributing to program development, and enhancing productivity through flexible collaboration.

Key Responsibilities:

• Perform assessments, develop treatment plans, and deliver therapeutic interventions.
• Participate in interdisciplinary team activities, quality improvement, and program development.
• Demonstrate accountability and contribute to problem solving and productivity enhancement.

Qualifications & Skills:

• Education: High school diploma required; a Doctorate of Physical Therapy (DPT) is required after 2015.
• Experience: Relevant experience with details on years and type of experience preferred.
• Certifications & Licenses: Licensed Physical Therapist, Basic Life Support (BLS), auto liability insurance, reliable transportation, and a valid Texas driver’s license required. Auto liability insurance must be obtained within 90 days of hire.
