Home Health Physical Therapist – Full Time

Position Overview

Seeking a Full-Time Physical Therapist (PT) to join a home health care team, enabling patients to recover at home. The role involves performing assessments, developing and implementing treatment plans, and documenting patient progress.

Key responsibilities include:

• Evaluating mobility, strength, and function through assessments
• Administering and interpreting tests to set goals and outcomes
• Creating and updating plans of care, including discharge planning and home exercise education
• Delivering therapy services according to physician orders
• Coordinating with healthcare teams and documenting effectively to ensure compliance
• Supervising personnel such as PT Assistants and Home Health Aides where applicable
• Completing assessments like OASIS, adhering to policies, regulations, and standards

Requirements:

• Degree in Physical Therapy from an accredited program
• Unrestricted state license as a Physical Therapist
• Valid CPR certification
