Role Overview: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and interventions, adhering to professional and ethical standards. The role involves contributing to program development, quality improvement, and interdisciplinary problem solving.
Key Responsibilities:
- Conduct evaluations and deliver treatments for patients across all ages, from adolescents to geriatrics.
- Implement therapies such as muscle strengthening, stretching, gait training, ADL training, and management of orthotic/prosthetic devices.
- Perform initial and ongoing patient evaluations, formulate treatment plans with goals, and monitor progress.
- Complete accurate clinical documentation in compliance with guidelines.
- Instruct patients and caregivers on activities, ensuring understanding through clear written and verbal instructions.
- Communicate regularly with healthcare providers regarding patient progress and plan adjustments.
- Manage appointment scheduling efficiently, including travel logistics.
- Maintain department policies, safety, and quality standards, and participate in departmental initiatives.
- Monitor insurance coverage and patient authorizations, report concerns to management.
- Perform additional duties as assigned.
Qualifications:
- High School Diploma or GED; Doctor of Physical Therapy (DPT) required, especially after 2015.
- Licensed Physical Therapist, valid Texas driver’s license, auto liability insurance, and BLS certification within 90 days.
- Individuals must have reliable transportation for the role.
