Physical Therapist Case Manager Job Description

Salary Range: $91,000 - $110,000 annually, depending on experience

Schedule: Monday – Friday 8:00am-5:00pm (weekend rotation: every 10th weekend / holiday rotation: 1 per year / no on-call rotation)

Position Functions & Responsibilities:

  • Provides physical therapy services to clients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:

+ Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

+ Assesses for muscle strength, mobility, gait, ROM—potential for rehab.

+ Directing physical therapy treatment.

+ Instructing clients and families/caregivers in the use and care of therapeutic appliances.

+ Determining priority needs for physical therapy.

+ Reporting to physician client’s reaction to treatment or changes in condition.

  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the client by participating in case conferences.
  • May train client in the use of prosthetic device.
  • Identifies client and family/caregiver needs for other home health services and refers as necessary.
  • Prepares and submits clinical and progress summaries based on the attainment of goals.
  • Participates in discharge planning for client.
  • Prepares and submits a clinical progress summary based on the attainment of goals as directed by organization policy.
  • Provides physical therapy consultation to home families/caregivers when indicated.
  • Provides in-service education programs for nursing organization personnel as needed.
  • Participates in peer consultation process.
  • Supervises physical therapy assistants according to organization policy and state regulations.
  • When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.
  • Lead by example in establishing rapport with clients.
  • Ensure positive/professional communication on team.
  • Customer service resolution; escalate concerns.
  • Honors client confidentiality, rights, privacy, and reporting maltreatment expectations.
  • Understands and accepts emotional needs of client.
  • Accepts unique client symptoms and behaviors.
  • Follow all Emergency and Response Procedures
  • Identifies home safety issues and notifies RN Case Manager or Clinical Manager of concerns or issues
  • Demonstrates proper procedures: Handling, lifting, transfers

Qualifications:

  • Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association.
  • Current and active Physical Therapist license in the state of Minnesota.

Physical Therapist Start of Care Job Description

Primarily responsible to plan, organize, direct home care services. Experience in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position

Start of Care/Admission Registered Nurse (RN) for Home Health

Wage range: $88,000-$90,000 annually, depending on experience

Service Area: Twin Cities Metro Area. Plus, mileage reimbursement of 72.5 cents per mile.

Schedule

Full-Time. 1.0 FTE / Monday – Friday, 8:00am-5:00pm (Rotations. 2 holidays per year, PRN on-call and no holidays)

Position Functions & Responsibilities:

  • Frame the discussion and builds understanding with clients that Lifespark starts with the clients’ immediate needs and wants (short term) while also working to build a longer-term plan to help the clients avoid the roller coaster of health care crisis™ thereby protecting and improving their independence and quality of life.
  • Effectively articulate and differentiate the Lifespark Life Care Management model.
  • Provide assurance and relief (peace of mind) to clients and families that Lifespark will effectively address their needs and wants.
  • Make the clients feel comfortable and well-cared for.
  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Initiates the plan of care
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development.
  • Completes necessary paperwork accurate at admission and submits to medical records the same day.
  • Completes emergency preparedness plan with client/caregiver at admission visit
  • Communicates with IDT post admission or non-admission including client status and a summary of the plan of care for client.
  • Ensure positive/professional communication on team
  • Introduction/Establish Rapport

  • Prepares clinical notes and updates the primary physician
  • Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders

  • Understands and Accepts Clients’ Diagnosis

  • Identifies home safety issues and cascades info to the IDT
  • Notifies IDT of emergency preparedness plan
  • Demonstrates proper procedures for handling, lifting, and transfers
  • Overview of Process, Types of Reports and Forms
  • Enters complaints, occurrences into EMR as necessary at skilled visits
  • Ensures education on how to use equipment safely is completed at admission and that medical supplies are ordered.
  • Plots necessary supervisory visits per regulations
  • Thoroughly reconciles all medications
  • Completes all documentation by 10am the day following the admission or skilled visit
  • Complete orientation and annual training requirements.
  • Promote safety in the workplace through real time coaching, participation in safety awareness training
  • Participate in After Hours on-call program
  • Other duties as assigned

Qualifications:

  • 2 years’ RN experience in health care (preferably home care or hospice)
  • Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferred
  • Experience in customer service/direct member contact field preferred
  • Experience supervising caregivers
  • Knowledge of Medicare Home Health & Hospice regulations
  • Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.
  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
  • Must pass a MVR check based on Lifespark company policy
