**Job Overview**
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a comprehensive understanding of physiotherapy practices, with a focus on delivering high\-quality patient care across diverse settings.
As a Physical Therapist, you will develop individualized treatment plans, collaborate with multidisciplinary teams, and support patients in achieving optimal functional outcomes. Your expertise in anatomy, physiology, and clinical documentation will be essential to providing effective rehabilitation services and enhancing patient well\-being.
**Duties**
- Assess patients’ physical conditions through detailed evaluations, including mobility, strength, and functional capacity.
- Develop personalized treatment plans that align with patient goals and clinical best practices.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs tailored to individual needs.
- Monitor patient progress regularly and adjust treatment strategies accordingly to ensure continuous improvement.
- Collaborate with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care plans and optimize recovery outcomes.
- Maintain accurate clinical documentation in accordance with healthcare standards and electronic health records (EHR) systems.
- Educate patients and their families on injury prevention, home exercises, and community care options to promote long\-term health.
- Manage discharge planning processes by evaluating patient readiness for transition to home or community settings.
**Requirements**
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments.
- Strong knowledge of anatomy, physiology, neurology, orthopedics, and geriatrics to inform treatment strategies.
- Familiarity with clinical documentation standards and electronic health records (EHR) management systems such as EMR platforms.
- Experience working with geriatric patients, pediatric populations, and individuals requiring neurological rehabilitation.
- Ability to develop comprehensive care plans that incorporate discharge planning and patient education components.
- Excellent communication skills for effective client interaction and interdisciplinary collaboration.
- Knowledge of physiotherapy techniques including trigger point therapy and physiotherapy modalities used in outpatient rehabilitation centers.
