Location: Miami Beach, FL — Patient homes

Starting Pay: $45 per visit

Legacy Health Care LLC is seeking a dependable, compassionate Registered Nurse to provide skilled nursing visits to patients in their homes throughout Miami Beach.

Responsibilities:

• Perform nursing assessments and monitor patient health status.
• Provide skilled nursing care according to physician orders and the established plan of care.
• Administer medications and treatments as ordered, within demonstrated competencies.
• Complete medication reconciliation and educate patients and caregivers on medication management.
• Teach disease management, infection prevention, and home safety.
• Recognize and promptly report changes in patient condition to the physician and agency.
• Complete accurate, timely documentation in WellSky/Kinnser.
• Complete OASIS assessments, admissions, recertifications, and discharges as assigned.
• Coordinate care with therapists, aides, physicians, and caregivers.

Qualifications:

• Active Florida RN license or valid multistate RN license authorizing practice in Florida.
• Current CPR certification.
• Reliable transportation to patient homes.
• Strong assessment, communication, and documentation skills.
• Home health, OASIS, and WellSky/Kinnser experience preferred.

Pay: $30.00 - $35.00 per hour

Expected hours: 7.0 – 14.0 per week
